Please find the attached scanned copy. I have faxed the application to the City Clerk's Office in Longmont. I spoke with Michelle Gomez and she has confirmed the receipt of the application form.
In addition we need the following:
1) Copy of the receipt for Lower Acre reservation
2) Site Plan
3) Form #7 (I will work on this)
4) Form #8 (I will work on this)
5) Neighborhood Notice (I will work on this)
6) Finally the $50 Fee
7) Certificate of Insurance
